What Are Whiplash-Associated Disorders?

Whiplash refers to a rapid acceleration–deceleration injury of the neck, often resulting from a rear-end car accident. This sudden movement can cause damage or irritation to the:

  • Cervical joints and discs
  • Muscles and ligaments of the neck and shoulders
  • Nerves in the upper cervical region
  • Temporomandibular joint (jaw)
  • Vestibular system (balance)


Whiplash-Associated Disorders (WAD) is the term used to describe the complex group of symptoms that can follow this type of trauma—ranging from mild discomfort to long-term disability.


Common Symptoms of Whiplash-Associated Disorder (WAD)

  • Persistent neck pain or stiffness
  • Headaches, especially at the base of the skull
  • Shoulder or upper back pain
  • Dizziness or light-headedness
  • Fatigue or difficulty concentrating
  • Jaw pain or clicking (TMJ dysfunction)
  • Tingling, weakness, or numbness in the arms or hands
  • Anxiety or fear of movement


These symptoms may appear immediately or days after the injury, and if left untreated, can become chronic or worsen with time.


The Graded System of Whiplash (WAD 0–IV)

Understanding your WAD classification helps guide appropriate treatment.

  • WAD 0: No physical signs or complaints
  • WAD I: Neck pain, stiffness, or tenderness only
  • WAD II: Neck symptoms + musculoskeletal signs (reduced ROM, point tenderness)
  • WAD III: Neck symptoms + neurological signs (reflex changes, weakness, sensation loss)
  • WAD IV: Neck symptoms + fracture or dislocation (requires urgent medical management)
