Refer a Summit when a claim has multiple, multifactorial factors and needs sustained support: regular case conferences with the treating team, ongoing measurement, and the full pathway through to recovery or a clear onward plan.

It is the tier with sustained support, up to three case conferences with the treating team, and the full pathway through to recovery or a clear onward plan.

Exactly what is in Summit

What is included

  • Everything in Vector
  • Two further Independent Clinical Reviews, about one every four weeks (three in total with Vector's), to track progress, build rapport and plan together with the treating provider
  • Four to eight further telehealth sessions, with a full Exercise Physiology transition and handover on the physical arm, or a structured handover on the psychological arm
  • Cognitive Coaching continued on the physical arm as the measures indicate
  • A closing review, with discharge or a clear onward plan

Both arms run at this tier. On the physical arm the work is steered to an Exercise Physiology transition with Cognitive Coaching available; on the psychological arm the treating psychologist leads, with physical exercise as the evidence based adjunct. Same engine, same price.

How Summit differs from the other two

Where it sits in the range

Summit is the complete pathway. On top of everything in Vector it adds sustained support and the run to recovery: up to three Independent Clinical Reviews in total, about one every four weeks, four to eight further sessions with a full Exercise Physiology transition and handover on the physical arm or a structured handover on the psychological arm, and a closing review that ends in discharge or a clear onward plan. It is for the complex, multifactorial claim, where Pulse or Vector alone would not be enough.
