When it has not gone away

For the pain that standard care has not settled

Persistent pain is different from a fresh injury. When something has been sore for months, the tissue is often only part of the story, and a quick appointment that treats one spot rarely gets to the bottom of it. That is the sort of problem we take on, the stubborn one that has already had a few goes at it.

We do not promise to make pain vanish, because honest physiotherapy does not work that way. What we offer is a proper look at what is keeping it going, a plan you actually understand, and steady progress you can measure. For many people that means real, lasting improvement in how much they can do and how they feel.

How we work

A thorough look at what is actually driving it

Your first appointment is a full 60 minutes, not a 20 minute rush. We look at how you move, how the pain behaves, what loads it up and what settles it, and the day to day things that quietly keep flare ups going. Then you get a clear explanation of what is going on and a plan built around it.

Treatment usually blends hands on therapy to settle symptoms with graded movement and exercise that rebuilds strength and confidence, plus practical education so you understand your own pain. Where muscle tension is part of the picture, trigger point dry needling can take the edge off enough to let the rehab do its job. The needle opens the door, the exercise keeps it open.

  • A full 60 minute first assessment, not a quick once over
  • A clear diagnosis and a plan you understand
  • Hands on therapy paired with graded exercise and education
  • Dry needling used where it helps, never as the whole plan
What it helps

The stubborn problems people bring us

Chronic pain physiotherapy is assessment led, so the plan fits the person rather than the label. These are some of the longer standing problems people come to us with. If yours is not here, the assessment tells us how we can help and whether we are the right people for it.

Who you will see

Led by Andrew Ellis

PACT was founded by Andrew Ellis, our principal physiotherapist. Andrew holds a Bachelor of Science in Exercise Science and a Master of Physiotherapy, brings more than 14 years of clinical experience, and has a long focus on trigger point dry needling as the physiotherapist behind @thedryneedlingphysio, where he teaches the technique to clinicians here and overseas.

That depth is what makes the difference on a complex problem, and it sets the standard for the whole clinic. Whoever you see at PACT works to the same approach Andrew built and treats alongside him, so you get the same careful assessment and the same reasoning behind the plan.

Getting here

In Miranda, or by telehealth

Our clinic is at Unit 1/196 Sylvania Road, Miranda, an easy run from across the Sutherland Shire, including Cronulla, Caringbah, Sutherland, Gymea and Sylvania. We are open Monday to Friday from 7:30am to 9pm and weekends from 8:30am to 6pm.

If getting in regularly is hard, PACT Virtual Care offers telehealth across Australia, which suits ongoing chronic pain care well because so much of it is guided movement, progression and education. Book online or call (02) 8330 6776.

Frequently asked questions

Good to know

Can physiotherapy help pain that has been there for months or years?
Often, yes, though honestly it depends on the problem and the person. Long standing pain usually needs more than treating one sore spot. A thorough assessment, a plan that addresses what is keeping the pain going, and steady graded progress helps many people improve how much they can do and how they feel. We will give you an honest view of what is realistic at your first visit.
What counts as chronic pain?
Pain that has lasted longer than about three months, or that keeps returning, is usually called persistent or chronic pain. By that stage the original tissue is often only part of the picture, which is why a broader assessment and plan tends to work better than chasing the sore spot alone.
What makes your approach different?
Two things. Time, because your first appointment is a full 60 minutes so we can assess properly rather than rush. And depth, because PACT is led by Andrew Ellis, an AHPRA registered physiotherapist with a long focus on complex pain and trigger point dry needling. You get a clear plan you understand, not a quick fix that fades by the weekend.
Do you use dry needling for chronic pain?
Sometimes, where muscle tension is part of the picture. Trigger point dry needling can settle symptoms enough to let the rehabilitation do its job, but it is one tool within a wider plan, never the whole treatment. The assessment tells us whether it is worth using for you.
Can I be treated by telehealth?
Yes. PACT Virtual Care offers telehealth physiotherapy across Australia, which suits ongoing chronic pain care well because so much of it is guided movement, progression and education. Many people combine telehealth reviews with occasional visits to the clinic.
How much does it cost and who will I see?
Initial physiotherapy is $165 for 60 minutes and follow ups are $142 for 40 minutes. Medicare Chronic Disease Management and private health rebates apply where you are eligible. You will see one of our physiotherapists, all of whom work to the approach Andrew Ellis built. Book online or call (02) 8330 6776.
