Disc bulge and herniation

Back pain, sometimes with leg symptoms, from an irritated or bulging spinal disc.

By Andrew Ellis, AHPRA registered physiotherapist · Reviewed July 2026

The condition

What it is

A disc bulge or herniation is when the soft cushion between two vertebrae pushes out and can irritate nearby structures or a nerve. It can cause back pain on its own, or back and leg pain together. Despite how it sounds, most disc problems settle well without surgery.

Signs

Signs and symptoms

Everyone is different, but these are common with disc bulge and herniation. You may have some and not others.

Back pain, often with bending or sittingPain or tingling into the legStiffness and muscle guardingWorse first thing in the morningPain with coughing or sneezingDifficulty standing straight
Treatment

How physiotherapy helps

We assess to understand how the disc is affecting you, then ease the pain and pressure with hands on therapy and carefully chosen movements. A graded program rebuilds strength and tolerance, and we guide your return to lifting and activity. Imaging and medical input are arranged only if warranted.

  • Pain easing treatment and positions
  • Graded movement and strengthening

Frequently asked questions

Good to know before you book

Does a disc bulge mean surgery?
Usually not. Most disc bulges settle with time and the right rehabilitation. Surgery is reserved for specific situations, which we help you recognise.
How much will it cost?
Initial physiotherapy is $165 for 60 minutes and follow ups are $142 for 40 minutes. Approved workers comp and CTP appointments are billed directly to the insurer.
Do I need a referral?
No referral is needed to book privately. Referrals are required for Medicare Chronic Disease Management plans and most claim pathways.
Can it be managed by telehealth?
Many problems can be assessed and guided well by telehealth through PACT Virtual Care, with hands on treatment added at the Miranda clinic when it helps.
