Ankle synovitis

Swelling, pain and stiffness in the ankle from an irritated, inflamed joint lining.

By Andrew Ellis, AHPRA registered physiotherapist · Reviewed July 2026

The condition

What it is

Ankle synovitis is irritation of the synovium, the soft lining inside the ankle joint. It often follows a sprain or a spell of overload, and it can also come alongside joint wear. The irritated lining makes extra fluid, which is why the ankle can feel swollen, warm and stiff, and why it can linger after the original injury seems to have settled.

Everyone is different, but these are common with ankle synovitis. You may have some and not others.

Treatment

How physiotherapy helps

We assess the ankle to confirm what is driving the irritation and rule out anything that needs more attention, then settle the swelling and pain with hands on therapy and gentle movement. A graded strengthening and balance program rebuilds the ankle, and we address the underlying cause, often an old sprain that was never fully rehabilitated, so it is less likely to flare again.

Frequently asked questions

Good to know before you book

What brings on ankle synovitis?
It often follows a sprain or a spell of overload, when the joint lining stays irritated and swollen. Settling the inflammation and then rebuilding the ankle's strength and control is usually what helps it calm down.
How much will it cost?
Initial physiotherapy is $165 for 60 minutes and follow ups are $142 for 40 minutes. Approved workers comp and CTP appointments are billed directly to the insurer.
Do I need a referral?
No referral is needed to book privately. Referrals are required for Medicare Chronic Disease Management plans and most claim pathways.
Can it be managed by telehealth?
Many problems can be assessed and guided well by telehealth through PACT Virtual Care, with hands on treatment added at the Miranda clinic when it helps.
