Achilles tendinopathy

Pain and stiffness in the Achilles tendon at the back of the ankle, worse in the morning.

By Andrew Ellis, AHPRA registered physiotherapist · Reviewed July 2026

The condition

What it is

Achilles tendinopathy is irritation of the tendon at the back of the ankle, where the calf attaches to the heel. It usually builds from running, walking or a change in activity, and is often stiff and sore first thing in the morning and after exercise.

Signs

Signs and symptoms

Everyone is different, but these are common with achilles tendinopathy. You may have some and not others.

Treatment

How physiotherapy helps

The Achilles responds well to graded loading. We confirm the diagnosis, settle the irritation, and build a progressive calf and tendon strengthening program. We manage your walking or running load so the tendon can rebuild its tolerance steadily.

Frequently asked questions

Good to know before you book

Why is it worse in the morning?
Tendons stiffen overnight. Morning stiffness that eases as you warm up is typical of tendinopathy, and a loading program is the mainstay of settling it.
How much will it cost?
Initial physiotherapy is $165 for 60 minutes and follow ups are $142 for 40 minutes. Approved workers comp and CTP appointments are billed directly to the insurer.
Do I need a referral?
No referral is needed to book privately. Referrals are required for Medicare Chronic Disease Management plans and most claim pathways.
Can it be managed by telehealth?
Many problems can be assessed and guided well by telehealth through PACT Virtual Care, with hands on treatment added at the Miranda clinic when it helps.
