Calf strains

A sudden grab or tightness in the calf, common with running and jumping.

By Andrew Ellis, AHPRA registered physiotherapist · Reviewed July 2026

The condition

What it is

A calf strain is a tear in one of the calf muscles, often felt as a sudden grab at the back of the lower leg during running, jumping or a quick push off. They range from minor to more significant and respond well to a graded rehabilitation program.

Signs

Signs and symptoms

Everyone is different, but these are common with calf strains. You may have some and not others.

Treatment

How physiotherapy helps

We assess the strain, settle it with hands on therapy and dry needling where useful, then progress a calf strengthening and loading program. A graded return to running or sport with clear milestones helps you get back without straining it again.

Frequently asked questions

Good to know before you book

How long until I can run again?
It depends on the grade of strain. We use strength and pain milestones to guide your return rather than a guessed date, so it holds up.
How much will it cost?
Initial physiotherapy is $165 for 60 minutes and follow ups are $142 for 40 minutes. Approved workers comp and CTP appointments are billed directly to the insurer.
Do I need a referral?
No referral is needed to book privately. Referrals are required for Medicare Chronic Disease Management plans and most claim pathways.
Can it be managed by telehealth?
Many problems can be assessed and guided well by telehealth through PACT Virtual Care, with hands on treatment added at the Miranda clinic when it helps.
